- Sunnyslope High School – Phoenix, AZ: Vision services & glasses request for a 17 yr old boy, referred to Nationwide. $74.99 to pay for an eye exam and glasses.
- Independence High School – Glendale, AZ: Vision services & glasses request for a 14 yr old boy, referred to Nationwide. $74.99 to pay for an eye exam and glasses.
- Heard Elementary – Phoenix, AZ: Vision services & glasses request for a 8 yr old boy, referred to Nationwide. $162.21 to pay for an eye exam and glasses.
