Large Grant Requests

Small Grant Requests
- Washington High School – Phoenix, AZ: Cardiologist appointment 18 yr old boy who needed clearance to play soccer and run cross country after his physical showed problems on an EKG and ECHO Gram. AFCCA Foundation for Children Board approved to pay the $102 cost for the appointment.
- Washington High School – Phoenix, AZ: Neurology appointment for a concussion in a 15 year old boy, referred to Banner Concussion Center. AFCCA Foundation for Children board approved paid $204.75 for the appointment.
